Care Blocks
Three- or six-month periods of structured physician management.
$299 per block
Each block includes:
- One scheduled physician follow-up and review of your lab results
- Treatment adjustments, prescription coordination, and refill management
- Direct messaging with your physician about your treatment
Laboratory testing and medications are billed separately.

A full-hour evaluation each year to look beyond your current treatment plan. We build on what’s working and decide what’s next.
Physician consultation: $229. Billed separately from Care Blocks. Laboratory testing is additional.
How it works
When: After your first year of care, then once a year.
What happens: You complete updated lab work before the visit. We spend a full hour reviewing your results, how you’ve felt, and the goals that matter to you now—and decide what comes next.
How it fits: It’s separate from your initial evaluation and your Care Blocks, which continue to cover ongoing treatment management.

Know the cost before you proceed. Your physician will explain the recommended testing, review medication options, and discuss costs with you before testing or treatment begins.
Follow-up labs and medications are billed separately from physician-care fees.

Testosterone medication

Depends on your prescription and pharmacy. Fertility support and other medications are priced separately.
Injectable GLP-1 therapy
$200–$330per four-week supply
This estimate is for compounded medication and varies by medication and dose. Higher doses may cost more.
Frequently asked questions.
Do I have to sign up for ongoing care before my evaluation?
No. Your Complete Evaluation stands on its own. If ongoing management or treatment is appropriate, you decide whether to continue with a Care Block.
When is my first Care Block billed?
Your first Care Block is billed when your initial prescription is sent, after you and your physician decide to begin treatment. It is not charged when you purchase your evaluation.
Is Forge a monthly subscription?
No. There is no monthly membership fee. Ongoing physician management is billed by Care Block, with laboratory testing and medications billed separately. The Annual Review is a separate consultation after your first year of care, then once a year.
Can I use recent outside lab work?
Possibly. Labs drawn within the past two months are reviewed individually. If they meet Forge’s testing requirements, your evaluation price may be reduced by the cost of the accepted tests. Missing or unsuitable testing will still need to be completed.
Can I use insurance?
Forge is a direct-pay practice. If you prefer to use insurance for laboratory testing, we can adjust your evaluation price and provide the appropriate orders and diagnosis codes. Coverage is not guaranteed, and your out-of-pocket cost may be higher than our cash lab price. Forge does not handle insurance approvals or appeals.
