These Terms apply to the public website. Separate agreements and notices may govern secure intake, telehealth, clinical care, payment, cancellation, prescriptions, patient communication, and other services.

Nothing on this page is a consent to treatment or telehealth, a financial agreement, or a waiver of rights relating to professional medical care.

The website is intended for adults age 18 and older.

Forge currently provides clinical services only to eligible patients who are physically located in Florida when care is delivered. Website availability outside Florida does not mean that Forge offers clinical services in another jurisdiction.

Website content is provided for general informational and educational purposes. It is not individualized medical advice, diagnosis, treatment, or a substitute for an appropriate evaluation by a qualified health care professional.

Do not use website content by itself to start or stop a medication, select a treatment, interpret personal laboratory results, or delay medical care.

Reading the website, submitting a public inquiry, receiving a general response, or following a link does not create a physician-patient relationship.

Completing intake or making a payment does not by itself guarantee acceptance for care or any particular service. A physician-patient relationship is established when Forge affirmatively accepts you for care and a Forge physician undertakes to provide professional services through the applicable clinical process.

The public website, ordinary email, and routine patient messaging are not emergency services and are not continuously monitored.

If you believe you are experiencing a medical emergency, call 911 or go to the nearest emergency department. Urgent symptoms or concerns may require prompt local, in-person evaluation.

Forge does not guarantee any diagnosis, laboratory order, prescription, medication, treatment plan, referral, or outcome.

Clinical decisions are made by the treating physician after an appropriate evaluation using the available history, symptoms, examination information when applicable, laboratory data, risks, benefits, alternatives, safety considerations, applicable law, and professional judgment.

References on the website to testosterone therapy, GLP-1 medications, sexual-health medications, fertility-support medications, supplements, laboratory testing, or other treatment options describe subjects that may be considered in appropriate cases. They are not recommendations for every patient and do not mean a particular treatment will be prescribed or continued.

Some published medication estimates and treatment descriptions may refer to medications prepared by a compounding pharmacy. Compounded medications are not FDA-approved or generic versions of FDA-approved drugs and are not reviewed by FDA for safety, effectiveness, or quality before marketing. Whether a compounded medication may be prescribed depends on the patient’s individual clinical needs, the treating physician’s judgment, applicable law and compounding requirements, and pharmacy availability.

Telehealth may not be appropriate for every concern. Forge may require in-person examination, imaging, emergency or urgent evaluation, primary care, specialty consultation, or transfer of care when clinically appropriate.

Forge makes reasonable efforts to keep published prices, service descriptions, and cost ranges current. Prices, services, availability, laboratory panels, medication options, and policies may change. The price and terms presented during checkout or in the applicable patient agreement control the specific purchase.

Forge is a self-pay practice. Forge does not submit insurance claims, verify benefits, seek prior authorization, or represent that Forge services will be reimbursed by a health plan. A pharmacy, laboratory, or other independent provider may have its own billing and insurance practices for charges it bills directly. Forge does not guarantee third-party coverage or reimbursement.

Medication, pharmacy, laboratory, imaging, consultation, and other third-party charges may be separate unless Forge expressly states that a particular charge is included.

Published prices are not a patient-specific Good Faith Estimate. Uninsured and self-pay individuals have rights described in Forge’s Good Faith Estimate Notice.

Ordinary email and the public inquiry form are not secure channels for medical information. If you provide contact information and request a response, Forge may use that information to respond and provide next steps, consistent with the Website Privacy Policy.

Established patients should use the secure communication method designated by Forge. Electronic communications can be delayed, misdirected, intercepted, or affected by technical failures. Do not use electronic messaging for emergencies.

The website may link to secure intake, messaging, scheduling, payment, laboratory, pharmacy, or other services operated by third parties. Those services have their own terms, privacy practices, availability, and technical requirements.

Forge is not responsible for outside content or services it does not control. A link does not imply endorsement of every statement, product, or service on the linked site.

You may not:

• attempt unauthorized access to the website, accounts, systems, or data;
• introduce malicious code or interfere with website operation or security;
• use automated tools in a manner that imposes an unreasonable burden or circumvents access controls;
• impersonate another person, submit false information, or misuse another person’s contact information;
• use the website for unlawful, fraudulent, abusive, or harmful activity; or
• copy or exploit website content for commercial purposes except as permitted by law or written authorization.

Forge may restrict access when reasonably necessary to protect the website, its users, or the practice.

Unless otherwise stated, Forge’s original website text, design, logo, graphics, and other materials are owned by or licensed to Forge and are protected by applicable intellectual-property laws.

You may view and print reasonable portions for personal, noncommercial informational use. No other license or right is granted.

Forge may change, suspend, or discontinue website content or functionality. Although Forge makes reasonable efforts to provide accurate information, the public website may contain errors, omissions, or outdated information and may be unavailable because of maintenance, technical problems, or events outside Forge’s control.

To the extent permitted by law, the public website is provided on an “as available” basis without warranties concerning uninterrupted operation, error-free content, or suitability for a particular non-clinical purpose.

To the extent permitted by law, Forge is not liable for indirect, incidental, special, or consequential loss arising solely from use of—or inability to use—the public website.

This limitation does not apply to professional medical services, alter the standard of care, waive duties imposed by law, or limit any right or remedy that cannot lawfully be limited.

Florida law governs these Terms, without regard to conflict-of-law principles. Any dispute arising solely from use of the public website or these Terms must be brought in a state or federal court with jurisdiction in Orange County, Florida, unless applicable law requires otherwise.

This paragraph does not govern professional-liability claims, patient-care disputes, or matters controlled by a separate patient agreement or mandatory law.

If a provision of these Terms is found unenforceable, the remaining provisions will remain in effect to the fullest extent permitted by law. A failure to enforce a provision on one occasion is not a waiver of the right to enforce it later.

Forge may update these Terms when its website, services, or legal obligations change. The effective date above identifies the current version. Continued use after an updated version is posted constitutes acceptance to the extent permitted by law.
